We aim to serve as an extension of the physician’s plan of care by providing a family-centered service devoted to the care of the sick and the elderly at home. We will evaluate the health problems and needs of the patient and provide home health services that will assist them in coping with their illness. The programs we have developed aims to encourage a maximum degree of independence for the patients and their caregivers. Our health care team will work closely with patients and caregiver in an effort to avoid complications and prevent further health deterioration. They will receive continuous education in the latest developments in health care so that Infinity Home Care Providers can maintain its commitment of providing skilled home health care to its clients in supportive environment.
